Questions

Why is my PDF so large?

Because phone photos are large, and they are embedded as they are. Run them through the compress image tool first, or compress the finished PDF, if it needs to be emailed.

Can it read HEIC straight off an iPhone?

Yes, HEIC is decoded on the way in, so you do not need to convert first.

Which page size should I use for a scanned form?

A4 outside North America, US Letter inside it. Fitted is better when the images are photographs meant to be looked at rather than printed.
